What is Discharge Planning?

 

Discharge planning is the service where your needs are assessed for after-hospital care, before you leave the hospital.

Discussing what you will need after you leave the hospital, allows you to share any concerns you may have about self care at home. Occasionally, a patient is discharged from the hospital and then he/she or family realizes that they are not prepared for care at home. Discharge planning can arrange for assistance in advance by providing patients Home Health services, special equipment, meal delivery, or home therapies like Physical Therapy.

The advantage to Discharge Planning is that you may be able to leave the hospital earlier, recover more comfortably, and make a smooth transition from the hospital to home with an appropriate plan of care.

Patient services has available information and access to the information given on the patient services page as well as other community resources for the benefit of the patient. This service applies to out of state assistance as well.
