Through this program MHCC is able to conduct a complete Health Risk Assessment (HRA) for qualified seniors and assist patients in scheduling annual wellness visits with a family practice provider.
An Annual Wellness Visit, which is 100% covered by Medicare once per year for Medicare patients, is specifically designed to determine a patient’s overall health risks and offers a personalized prevention plan to help keep our senior patients in optimal health year-round!
